New U.S. Passport
Requirements to Expedite
- Effective May 22, 2006 all DS-11 passport applications submitted to the US Passport Agency from Peninsula Visa Service must be completed on line, downloaded and printed from the internet, and contain the (2D) barcode on the left side of the form. (Create your new DS-11 here ).
The preceding link is provided by the USA State Department whose software only supports Internet Explorer or Mozilla Firefox browsers.
For maximum timesaving benefit, the 2-D bar coded pages must be clear and sharp in black print on white paper. The paper must be 81/2 inches by 11 inches, with no perforations. The paper must be at least medium weight (20 lbs.), with a matte surface. Applications printed on thermal paper, dye-sublimation paper, any glossy paper, or on a dot matrix printer will not be accepted.
Applicants who submit executed hand written DS-11's in sealed envelopes with Peninsula Visa must fill out a new 2-D bar coded page one only of the DS-11 and attach it to the outside of the envelope which remains sealed.
- Two Passport photographs, taken within the past 3 months.
- Proof of U.S. Citizenship: a) certified birth certificate, issued by state or county
health department or vital statistics bureau (not hospital certificates); b)
naturalization certificate; ac) certificate of citizenship; d) Consular report of birth

- Proof of ID must be enclosed. An original or photocopy of one of the following must be enclosed:
Previously issued, undamaged U.S. passport, or Naturalization Certificate, or Valid Driver's License, or Current Government ID (city, state or federal), or Current Military ID (military and dependents)
- Letter of Authorization, "Attn: U.S. Passport Agency" stating:
"I, (applicant name), hereby authorize Peninsula Visa and Passport Service to pick up
my passport, and to discuss the status of my application with the Passport Services

- Proof of departure date is required (copy of itinerary or air tickets showing applicant
is departing the U.S.).
- Take the above requirements to your local acceptance
agent for Passport Applications (usually a post office or county
clerk or court house). Pay the clerk the U.S. government fee $195.00 total (2 checks $170 for the US State Dept and $25 for whomever seals the package)
for age 16 and older, $165.00 for children under 16 years(2 checks $140 for the US State Dept,and $25 for whomever seals the package). Effective February
1, 2008 both parents and the child must be present when submitting applications
for minors (birth through 16 years of age). If only
one parent can appear,then
one parent appears, signs, and submits second parent's notarized statement of consent authorizing passport issuance for the child (a notarized Form DS-3053, Statement of Consent: Issuance of a Passport to a Minor Under Age 16, or a notarized written statement with the same information on a sheet of paper from the non-appearing parent or Primary evidence of sole authority to apply, for example,
a Court order granting sole custody to the applying parent (unless child's travel is restricted by that order) or
an
Adoption decree (if applying parents is sole adopting parent). Ask the clerk to execute the
documents and place them in a sealed envelope addressed to "U.S. Passport

Processing Instructions
Please deliver to our office or call for
pickup. If out of the area please forward by Federal Express and
call 1-800-665-8472 with departure information and Fed-Ex tracking
number. This location will accept Federal Express packages only you must ship "Priority Overnight",mark Hold for pickup weekdays at bottom of Federal Express Air bill. You must call
Peninsula Visa the day you ship your documents and get
a work order number. At that time your order will be
taken. If you do not call there may be no courier available to process your
package. Also we will process your documents as RUSH service
and additional costs will apply
if you do not have a work order number.

Processing Information
Your application will be hand carried to the Passport Agency in San Francisco Ca. There,
the Passport Agent will examine the application and if all documents are in order, assign
a date the passport will be issued, according to departure date from the U.S.
Passport applications submitted by mail to passport agencies routinely take 4 to 6 weeks
to be processed. PVPS expedites applications by hand carrying them to the U.S. Passport
Agency, and following up on the progress. Please note that PVPS cannot offer any
guarantees of issue date.
A 48-page business travelers passport can be issued in lieu of the usual 24-page
passport on request at no additional charge. Attach a note requesting a 48-page passport
to the passport application.
