Passport
Requirements to Expedite:
Effective May 22, 2006 all DS-11 passport applications submitted to the US Passport Agency from Peninsula Visa Service must be completed on line, downloaded and printed from the internet, and contain the (2D) barcode on the left side of the form.(Create your New DS-11
here)
The preceding link is provided by the USA State Department whose software only supports Internet Explorer or Mozilla Firefox browsers.
For maximum timesaving benefit, the 2-D bar coded pages must be clear and sharp in black print on white paper. The paper must be 81/2 inches by 11 inches, with no perforations. The paper must be at least medium weight (20 lbs.), with a matte surface. Applications printed on thermal paper, dye-sublimation paper, any glossy paper, or on a dot matrix printer will not be accepted.
- Completed form DS11 plus a DS-64, Statement Regarding Lost or
Stolen Passport. Obtain these forms at a post office or county clerk's office, or print DS-64 from the
Internet.
- Two Passport photographs, taken within the past 3 months.
- Proof of U.S. Citizenship: a) certified birth certificate, issued by state or county
health department or vital statistics bureau (not hospital certificates); b)
naturalization certificate; c) certificate of citizenship; d) previous U.S. passport.
- Proof of Identity, with photo: a) valid driver's license, b) valid college
identification card, c) current military or government i.d. card, d) a Certificate of
Naturalization or Citizenship, e) a previous U.S. passport, f) corporate or employer
I.D.
card.
- Take the above requirements to your local acceptance agent for Passport Applications (usually a post office or
county clerk or court house). Pay the clerk the U.S. government fee ($170.00 for adults age 16
and older, $140.00 for children under age 16,plus a $25 fee for whomever executes the application). Ask the clerk to execute the
documents and place them in a sealed envelope addressed to "U.S. Passport
Agency". Letter "Attn: U.S. Passport Agency" stating: "I, (applicant name),
hereby authorize Peninsula Visa Service to pick up my passport, and to discuss the status
of my application with the Passport Services staff." Print
letter.
- Proof of departure date is required (copy of itinerary or air tickets showing applicant
is departing the U.S.).
Processing Instructions
Please call 1-800-665-8472 with departure and Fed-Ex tracking information. This
location accepts packages by Fed-Ex only. You must mark Hold for Pickup Weekdays at bottom of Fed-Ex
air bill. You must call Peninsula Visa the day you ship your documents and get a
work order number. At that time your order will be taken. If you do not call there may be no courier available
to process your documents. Also we will process your documents as
RUSH service and additional costs will apply if we receive
your documents without a work order number.
